Where Our Story Begins:
The Reason We Built New Hope

New Hope was founded by husband and wife team Zeshan and Nareesa, rooted in a shared belief that primary care should feel more human. After years spent inside traditional healthcare systems, they saw how often care becomes rushed, fragmented, and impersonal. Patients feel unseen. Providers feel stretched thin. Somewhere along the way, the simple act of listening began to disappear.

New Hope grew from a question they kept coming back to: What would care look like if relationships truly came first?

At New Hope, care begins with time. Time to talk, to understand, and to know the person behind the chart. Visits are designed as conversations, not transactions, and relationships are built to last, not reset with every appointment.

The name New Hope reflects both optimism and intention. Hope for a different experience for patients. Hope for a more sustainable and fulfilling way to practice medicine. And hope that care, when rooted in trust and presence, can truly support people through every stage of life.

Our office is designed to feel more like a calm, welcoming space than a traditional medical clinic. With no reception desk, medical assistants, or scribes, the environment is intentionally simple, allowing for a more personal and direct experience with your physician.

More Time Focused on You

You are never rushed here. Appointments at New Hope are intentionally 25% longer, so visits are calm, unhurried, and centered on you. By completing forms ahead of your visit, we spend less time on paperwork and more time understanding your health.

Complimentary Telemedicine Calls

We include a complimentary first virtual call, along with up to four free telemedicine calls each month, as part of your care. Monthly telemedicine calls are available to patients seen within the past 90 days and are typically offered within 72 hours of your request.

Faster Follow-Up When You Need It

After a recent visit to the hospital, emergency room, or urgent care, timely follow-up can make a big difference in recovery. Reach out and let us know your situation, and we will prioritize getting you in for a visit as soon as we can.

We use AI and digital tools to support your care and reduce administrative tasks, so we can stay focused on you. These tools may assist with documentation or monitoring, and always follow strict privacy and HIPAA standards.

Following years as a healthcare executive and federal advisor, Dr. Zeshan Rajput realized the most impactful change happens in the exam room. He founded New Hope Primary Care to move beyond traditional corporate models and create a more patient-centered experience.
An Internal Medicine physician with advanced training in Clinical Informatics, Dr. Rajput combines cutting-edge innovation with a deeply personal approach, ensuring every patient in Howard County feels heard, understood, and empowered.

After years as a bedside physician, healthcare executive, and federal policy advisor, Dr. Nareesa Mohammed-Rajput discovered that her true passion lies in caring for people on a more personal level. She co-founded New Hope Primary Care to step away from corporate medicine and return to a more genuinely patient-centered approach.

Board-certified in Internal Medicine with advanced training in Clinical Informatics, Dr. Mohammed-Rajput blends modern technology with a warm, personable style. She is especially focused on helping patients build practical, sustainable habits that support long-term health and allow them to become the best versions of themselves.
